Current Davis High School Students
Please print and fill out the Transcript Request Form, as well as pay the $2.00 per copy in the Main Office. Return the completed form and receipt to the Counseling Center for processing.
AP, ACT and SAT scores will be included with all transcripts
All requests must be completed and signed
Current students may also use Parchment.com, fees will apply.
Alumni Davis High Students
Alumni and third-party vendors must use Parchment for transcript and education verification requests.
